Coordinated Entry Data Quality Report

Coordinated Entry Program data that has been added to HMIS by your program. HUD COC-funded Programs will be listed below individually, however, the actual report uploaded to SAGE will be a “System-Wide ” Report which includes all the programs on our system. 

If any issues with the report, please contact us at hmisdept@vialink.org. This report is ran for the specific program grant start date to current. Users are encouraged to collect all data for the Coordinated Entry APR. Note: minimal data is needed for the System Performance Measures as required for the Coordinated Entry programs. 

Please remember to run both Reports to get the data needed to fill out the UNITY’s Quarterly forms. If your data does not look accurate, it is advised to review two area. In addition, to running your report from Grant Start Date to End. 

2 Key Things

  1. Coordinated Entry Assessment should be done within the grant reporting period. (Remember for our Carryover Clients that you must add a New Coordinated Entry Assessment with the program Start to End date).
  2.  Make sure you have identified the Head of household and the HOH family members in the actual assessment.
